Bridges to Success
Bridges to Success is a weekly social skill building and goal based program for young adults with autism ages 16-24 who require higher levels of support. The program focuses on life, recreation and social skill development that promotes community exploration and engagement. This program is designed to be a bridge program between other Autism Nova Scotia services; including Person Directed Planning Program (PDPP). This program requires participants to attend with a support worker/respite worker. Respite workers will learn support tools to better support participants in other settings at reaching their goals outlined in their PATH.

Although Bridges to Success has limited spots at any given time; if you are placed on a waitlist, as soon as another participant completes their 8 week program, our coordinators will reach out to you to onboard you into the program.
Once you have submitted your application, one of our facilitators will be in contact with you to schedule an intake. During the intake process, if it is identified that Bridges to Success may not be the right program for you, our coordinators will support you in getting connected and referred to other options.

Note: This program requires the presence of a support worker/respite worker in attendance with each participant.
Our facilitators will work with the support workers during sessions to develop their skill set in supporting participants in skill building and steps of their goals. This will allow for a full circle of support approach as skills can continue to be worked on at home and in the community.

Note: There is a heavy emphasis on community exploration, many sessions will be held in the community. This is not a drop in program. Participants must register with our Bridges to Success Facilitators and receive confirmation prior to attending the program.
Description:
Bridges to Success is a collaborative program delivered by Autism Nova Scotia support staff, and is supported by the individuals support/respite worker. It is a program for autistic youth and young adults who require 1:1 support from a respite worker or caregiver who will attend sessions with them to support the skill building development and taking steps towards a personal goal.
Bridges to Success is a person directed, individualized program. Each of our facilitatrs are paired with participants & their support worker to help identify personal goals, and develop in collaboration; sessions that take steps towards that goal. Participants will also have the opportunity for bi-weekly group sessions to ensure that social connection is kept at the forefront of our program delivery.
This program will help participants work towards their goals by creating person-driven plans, and supporting each participant in their transition into meaningful community-based social and recreational activities.
