Person-Directed Planning Program (PDPP)

Person Directed Planning is a process of imagining what a person’s idea of a positive and possible future looks like and then bringing together the resources and connections that will help them to achieve those goals. The Person-Directed Planning Program works with people to discover and work towards their personal vision of a positive and meaningful life.


Ages/Life Stage:

Young Adults (16 and up)
Adults (18+)

To be eligible for this program:

  • You must live in the Halifax Regional Municipality or Western Nova Scotia (including the counties Hants, Kings, Annapolis, Digby, Yarmouth, Shelburne, Queens, and Lunenburg)
  • You must not be currently enrolled in a supported residential or day program
  • Anyone who self identifies or is diagnosed with a disability, mental health concern, ADHD, or Autism is eligible

Description:

Person Directed Planning is a process of imagining what a person’s idea of a positive and possible future looks like and then bringing together the resources and connections that will help them to achieve those goals.

The Person-Directed Planning Program works with people to discover and work towards their personal vision of a positive and meaningful life. Participants work with a Person-Directed Planning Facilitator to identify 1-3 goals that reflect their values. Together, the goals are broken down into manageable steps that can be worked on individually, with community resources and/or other supports.

Participants may create goals one or more of the following areas; Housing, Recreation and Leisure, Employment and Volunteering, Community Exploration, Self-Regulation, Social Connection and/or Life Skills

​Autism Nova Scotia’s Person-Directed Planning Program is a collaborative and community-oriented approach to building a more inclusive and supportive society.

To express interest in the Person Directed Planning Program, please fill out the form.

Once you submit the form, someone from Autism Nova Scotia will contact you to confirm they have received it.

​Please note:  

Program registration is limited. All completed forms are time-stamped based upon arrival and processed in that order. Because of the high volume of interested people in this program, we cannot hold spots. Completing a pre-screening form does not guarantee you a spot in the program; you will be contacted within 2 days of filling out this form to talk more about the program.
